Comparative Efficacy of Intravenous Levetiracetam and Phenytoin in Status Epilepticus
Main Article Content
Abstract
Background: The literature surrounding the comparative efficacy of intravenous levetiracetam and phenytoin in managing status epilepticus (SE) reveals a significant shift in treatment paradigms, particularly in pediatric populations. Literature Review: (Tiamkao et al., 2013) laid the groundwork by emphasizing the importance of timely intervention in SE and suggesting that newer antiepileptic drugs (AEDs) like levetiracetam could provide additional benefits compared to traditional therapies (Tiamkao et al., 2013). Subsequent studies, such as that by demonstrated that intravenous levetiracetam was both efficacious and safe in pediatric patients, further supporting its role in acute seizure management . The EcLiPSE study protocol by (D Lyttle et al., 2017) aimed to rigorously compare levetiracetam and phenytoin, highlighting the potential for levetiracetam to be favored due to its rapid infusion capability and lower incidence of adverse effects (D Lyttle et al., 2017). The findings from the EcLiPSE trial (D Lyttle et al., 2019) confirmed that while both medications had comparable efficacy and safety profiles, levetiracetam demonstrated advantages in terms of ease of administration and fewer adverse reactions (D Lyttle et al., 2019). This was echoed by (Noureen et al., 2019) and (Wani et al., 2019), who reinforced the clinical efficacy and safety of levetiracetam in managing generalized convulsive SE, highlighting the need for direct comparisons between the two drugs [ref: 440faa9a-277f-409d-a0c5-9c30cc7ff3b1; ref: 66a34099-b5b6-4ca4-9a80-583c5fe7f440]. Conclusion: In conclusion, the literature indicates a paradigm shift in the management of status epilepticus, particularly in pediatric populations. While phenytoin remains a staple treatment, levetiracetam emerges as a viable alternative with potential benefits in safety and administration. Continued exploration through rigorous clinical trials is essential to further substantiate these findings and refine treatment protocols for SE.
